[Wtpcommittee] Draft Renewal Application Form

Attached is a further revision of the draft Renewal Application form for use
when renewing the WTP designation. Please see the redlined changes. Note the
deletion of the 2nd signature on page 2, and the addition of payment options
on page 3. Additional changes/further revisions/unrevisions welcomed.
